Born: June 25, 1934, Los Angeles, California.
Jack was the son of Jack Hayford and Anita Delores Farnsworth. He married twice, to Anna Smith (1955) and Valerie Lemire (2018).
He graduated from L.I.F.E. Bible College (now Life Pacific University), Los Angeles (1956) and Azusa Pacific University, Azusa, California (1970).
He was the national youth director of the International Church of the Foursquare Gospel (1956–73).
In 1965, he joined the faculty of L.I.F.E. Bible College. He later became dean of students there, and its president in 1977.
In 1969, he was the founding pastor at Church of the Way, Van Nuys, California. Starting with 18 members, the church eventually grew to a congregation of over 10,000.
