Hez was the son of Edmund Bowers Swem, and husband of Minnie Lee.
Ordained in March 1881 in Lawrenceburg, Indiana, he had his first pastorate there.
He moved to Greensburg, Indiana, around 1884 to become pastor of the Second Baptist Church, a post he held over 25 years.
He later served as pastor in Washington, DC, at the Centennial Baptist Church and the Second Baptist Church, leaving in 1910 for a pastorate in Dotham, Alabama.
Swem was a well known lecturer on the Chautauqua circuit.
